New Through The Gate service for YSS YSS is delighted to be working in partnership with the Warwickshire andWest Mercia Community Rehabilitation Company (WWMCRC) in deliveringan innovative Through the Gate service in HMP Hewell and HMPFeatherstone. This new initiative will address the individuals’ needs usingthe seven reducing re-offending pathways and includes both pre and postrelease support. The Re-Connect Prison Link Workers will provide • Six one to one sessions to any individual within the prison setting whorequires additional support focusing on wellbeing, emotional and physicalhealth and personal relationships, preparing them for their release back into the community.• Tailored support to the individual’s needs.• Signposting to other resources within the prison where required.Individuals being released to Worcestershire, Herefordshire, Shropshire,Telford and Warwickshire requiring additional support, on their release may bereferred to the Re-Connect Community Link Worker for post-release support. The Re-connect Community Link Worker will provide • A minimum of one pre-release visit.• Gate collection on the day of release.• Transport to their local community.• Eight community based sessions at times and places convenient to theservice user. The one to one support will focus on the seven life pathways:accommodation, children and family, finance and debt, education, trainingand employment, attitude, thinking and behaviour, health, including mentalhealth and substance misuse. Referrals can only be made to the Re-Connect service by the individual’sWWMCRC resettlement team and community based Offender Managers.
