YSS is delighted to be working in partnership with the Warwickshire and
West Mercia Community Rehabilitation Company (WWMCRC) in delivering
an innovative Through the Gate service in HMP Hewell and HMP
Featherstone. This new initiative will address the individuals’ needs using
the seven reducing re-offending pathways and includes both pre and post
release support.

The Re-Connect Prison Link Workers will provide

• Six one to one sessions to any individual within the prison setting who
requires additional support focusing on wellbeing, emotional and physical
health and personal relationships, preparing them for their release back in
to the community.
• Tailored support to the individual’s needs.
• Signposting to other resources within the prison where required.
Individuals being released to Worcestershire, Herefordshire, Shropshire,
Telford and Warwickshire requiring additional support, on their release may be
referred to the Re-Connect Community Link Worker for post-release support.

The Re-connect Community Link Worker will provide

• A minimum of one pre-release visit.
• Gate collection on the day of release.
• Transport to their local community.
• Eight community based sessions at times and places convenient to the
service user. The one to one support will focus on the seven life pathways:
accommodation, children and family, finance and debt, education, training
and employment, attitude, thinking and behaviour, health, including mental
health and substance misuse.

Referrals can only be made to the Re-Connect service by the individual’s
WWMCRC resettlement team and community based Offender Managers.
