What is Re-Connect? 

YSS is working with the Warwickshire and West Mercia Community Rehabilitation Company (WWMCRC) to deliver Re-Connect, a new Through The Gate enhance service in HMP Hewell and HMP Featherstone, as well as in the community in Worcestershire, Herefordshire, Shropshire and Telford.

The Re-Connect Prison Link Worker provides six one to one sessions for any individual within the prison setting who requires additional support, focusing on wellbeing, emotional and physical health and personal relationships, preparing them for their release back in to the community. They also offer tailored support and signposting to other resources within the prison as required. Individuals being released to Worcestershire, Herefordshire, Shropshire, Telford and Warwickshire requiring additional support on their release may be referred to the Re-Connect Community Link Worker for post-release support.

The Re-Connect Community Link worker provides a minimum of one pre-release visit, gate collection on the day of release, transport to the person's local community and eight community based sessions at times and places convenient to the individual. The one to one support focuses on the seven life pathways: accommodation, children and family, finance and debt, education, training and employment, attitude, thinking and behaviour, health - including mental health - and substance misuse.
